creating telelporters, help

Hi all , i did a search and i couln't find anything that really helped. I am trying to make a teleporter, it doesn't seem to be quite the same as radiant and the like so a bit of help would be great. I placed a point_teleport and an info_teleport_destination, i couldn't find any other teleport entities, I've messed around with the output stuff a bit, but can't seem to make it work. What am i missing? Thanks..!!
