CS:S video initialisation error

Just reported this bug through CS:S, and am wondering whether anyone else here has seen it?

About 90% of the time I have trouble starting up CS Source.
I get a "failed to initialize video - resetting to defaults" message. If I select Ok it always fails again and re-displays the message. Selecting Ok loads the game in an 800x600 window.
If I now change my video settings to 1024x768 full screen the game crashes with an exception.
As I say, once I have got the first error it always ends up in the crash, so I have to get lucky first time around...
I have XP Pro SP2, a Radeon 9800 Pro (128Mb) with up-to-date drivers, and DirectX 9.0c.
All my mobo drivers are up-to-date aswell.

I think the problem may be related to the fact that I am running a dual monitor setup. My primary display uses the DVI out on the card, my secondary uses the analog out.
I really hope you guys have some ideas because it's quite a annoying problem, and if I have trouble starting HL2 I'm going to be gutted :(

The game runs like a dream when it's runs ok, I get 102 fps in the stress test. I must say I'm incredibly impressed with the performance you've got the Source engine running at. I owe you all a beer :)

Has anyone else been having this problem?
 
I updated my drivers to cat 4.10 and it kept saying "failed to initialize video" so I rolled back to 4.9 and it was all fine. Maybe I did something wrong like not uninstall 4.9 the right way, but give it a shot..
 
Hmmm, I never uninstall the old Cats, just install over the top.
Never had any problems with other games though...

Cheers though, I'll try it later.
