WARNING: SPOILERS
It's an ending stutter, when breen shoots you with the enhanced Gravity Gun. It just goes BANGBANGBANGBANG *WINDOWS SOUND* and crashes to desktop.
Do i need to update files via source? I had the impression Source did this for me automatically.
(PS i've played through the game already, this only happened the 3rd time)
(PPS You think it might have anything to do with typing ent_fire breen ignite?)
