Fog and skyboxes.

I apologize if this has been asked before, but fog is too short a word for the forum's search function.

If you have a map with a 3D skybox, how exactly should you create fog that matches? I know you can set the skybox's fog with the camera settings, but then do you also need a env_fog_controller in the main part of the level? Do you need to multiply the distances by 16?

Using EntEd, I see that in at least one map, Valve managed to even put some fog settings into Worldspawn, even though the settings aren't there in Hammer.
