what does it mean really, the specs u need to run the game on max smoothly or what?

I think the recommended specification means what it the software needs to run at a decent frame rate on medium detail. I always take the recommended spec to be the minimum required as anything lower will usually only just run it.

Minimum specification is usually what you need to get the game running, while preferred specs are what you need to have a good time playing it.
