What are the router ports I need to open to host CSS?

You need to forward port 27015 to the IP address of the PC your are making a dedicated server, I suggest you set that PC up with a static IP as well.

If you have service pack 2 then you can either disable the firewall, I would suggest this for the server, or you can forward the ports just like you did on the router.
