Screen blanks - VisionTek Xtasy Radeon 9600 256

I just installed a new VisionTek Xtasy Radeon 9600 256 from CompUSA, and it seems ok until I start to play HL2 -- I can see the menus and load a saved game, but once I actually start playing, the screen goes blank and I must reboot. This also happens when I run the Direct3d 9 test in DxDiag. Problem occurs with ATI CAT drivers 4.11 and 4.12 beta. Otherwise card seems ok. Previous card GeForce2 MX had no problem in HL2, but I wanted better peformance.

System board: Intel 850EMV2 with latest BIOS
CPU: Intel P4 2.4
RAM: 512
Monitor: Cheap LCD connected with 30-foot KVM cable.

Any ideas? Do I need to change some settings? Is my card defective?

Have you used Driver Cleaner software to get rid of the remnants of the old Nvidia drivers?

I tried Driver Cleaner today and it did not help. In fact, now the screen blank problem occurs during the first Direct3D test in DxDiag. Previously it passed 7 and 8, but went blank during 9. Today it went blank during 7.

Once screen goes blank, I must reset PC. What other troubleshooting can I do?
