Smoke/Grenade Problem

I've got a problem whereby when I throw a grenade, or a smoke grenade, the smoke won't emit and only a brief flash of light will appear when the grenade is thrown. So basically, the smoke grenade will have no smoke emitting, as with the grenade (no explosion). Can you tell me how to fix this?

Oh and muzzle flashes do not appear as well.
