Dual Monitor Setup + fan problem

I just hooked up my old Dell monitor to my 9600xt. I really like this dual monitor setup. The only bad thing I don't know how long it will last because I think the Dell monitor is going to die soon, the color seems to be fading a little.

On to the question. With the 9600xt, it came with this Smart Doctor program that monitors voltage, fan speed, and temp. But every once in a while it comes up with this error that my gpu fan has stopped, I even opened up the case to see and yes, it was stopping. Now this only happens once in a while. Does this mean it is a bad card or what?
 
The fan might be bad. If you just bought it, I would exchange it.
Or you can buy a vga cooler to replace it.
I bought a gf4 4200 from a friend of mine who had it just sitting around and the fan on it did the same thing.
I noticed it had stopped every once in awhile. I just replaced the VGA cooler with a quality copper cooler.
 
It only did it when I had the BIOS BUS frequency at 133/33, when I have it at 100/33 it runs fine.

I went and changed the frequency jumpers on the mobo to run the cpu and ram at 133, so far I think its working because normally when I had just changed the frequency in BIOS my internet connection program wouldn't work but now it is.

Computers, you love to hate them.
