There was this show in the 80s called St. Elsewhere, about a hospital. In the series finale, the entire show turned out to be the imagination of an autistic boy named Tommy Westphall who had a snowglobe with the St. Elsewhere hospital in it.

The picture is playing off that, in that Frankie Foster is autistic and all of the characters (save for her grandma) and scenarios that happened in the show were figments of her imagination.
