Insane Lag on all Servers

I've set all graphics settings to minimum, and I get stellar performance offline. With this current lag the game is basically unplayable. Help?

Lag is a connection problem and should not be affected by your graphics settings. What is your actual problem? Is it you have a poor connection (high ping) or you have bad performance (low frames per second)?

net_graph will tell you everything you need to know. Open up the console (~) and type in that command. To disable it, use 0 instead. Look at FPS, ping, and choke.

Was it across a number of servers? Some servers are teh sketch. I haven't noticed a game-wide lag myself.

I play on servers with a latency of 9, with 40-50 fps, yet the game freezes every five seconds.
