I finally got mine working. This is what I did so hopefully it will help others. In the config.cfg I set the following settings:
snd_mixahead "0.4"
sv_forcepreload "1"
cl_forcepreload "1"

I created an autoexec.cfg and put this line in it:
cl_smooth "0"

I went to the launch options for HL2 and put this in:
-heapsize 384000

Next I went to my video card settings in the windows control panel. I changed all of the 3d settings to not be application preference and then set the aa and af to 2x. Since I have an ati card I also changed the Texture and Mipmap Detail Level settings to the second mark which is performance.

In the game settings I changed the audio to medium and the video from trilinear to bilinear. After doing all of this I get no stutter from the sound or the gameplay. I have been in several battles with things going and it works. The graphics are still good. I hope this helps someone else.

I did basically the same thing. It helped, but it isn't a 100% fix for me.

That's a lot of tweaking how bad was your stuttering? I Have a 1.8 gig & Nvidia ti4200 I experience some stutter. It almost feels like a harddrive thing. Like it's trying to load stuff everynow and then. But then again I have a lot of the settings turned up to high and it's running in 1024x768 with AA 2x and I believe trillinear filtering. What amazes me is that the game actually runs at those settings. But then I read somewhere else that with that video card it's not turning the better graphics on.

I member trying to turn the reflectivity on water to a higher level and not seeing a diff. Also with the settings cranked I don't see that really good looking facial features like oily skin or the way it kinda looks porus. I am not really complaining it still looks good and runs well most of the time but every now and then for no reason it stutters.

I did update my drivers. Who knows what the prob is but I feel like this game needs some more tweeking by Valve and me.

a lot of stuttering

I had a lot of stuttering. I have a 2.7ghz cpu and a Radeon 9800 pro with 2 maxtor 120 sata drives in a raid 0 with 1gb pc3500 high speed ram. Sound is nvidia onboard soundstorm. The only way I could get it to work was by doing all of these tweaks at once.

I honestly don't think it's an issue that can be fixed with tweaks, more likely a genuine programming issue. For me it stutters the same with or without these tweaks, seems the same. And it happens ALOT like every few seconds at the start in the train station!!

I recently got a "error memory could not be read" bug which I fixed by renaming a file "source models.gcf" and the redownloading it from steam which fixed the above bug but now the game stutters constantly. It didnt used to stutter before the fix.Btw I am and was running with mostly high settings and 2x ant-aliasing.
