It was Contra for the NES. For 2 players to get 30 extra lives. I remember playing this game almost every day during my freshman year in High School.

It was cheat from Jedi Knight II: Jedi Outcast. It turned on, well, realistic lightsaber effects, so if you slashed at an enemy, depending on where you hit him, chunks/limbs would be cut off. It added a lot of fun to game, not to mention realism, and made it feel much more like an actual lightsaber.
Here's a picture from the Jedi Academy game, but I don't remember there being this much blood (turns out that this pic is from a mod, so the blood is added). I know in Outcast there was no blood, it was identical to an actual lightsaber wound.
