Intersex people are born with both genitalia. Shemales are males who undergo a sex change but never have genital surgery, and are typically associated with pornography or prostitution. Transsexuals have genital surgery, and are different from shemales in that they have been legitimately diagnosed with gender identity disorder.

many transexuals cannot afford surgery, or have been deemed unfit for surgery by a doctor. They have been diagnosed by a psychiatrist, but either choose not to/ cannot afford sexual reasignment surgery.
