I was watching somebody play the other day, he was at the beginning of Follow Freeman I believe, and a Combine soldier came around the corner and got snagged by a barnacle. We both watched as he went up, and up, and up.... and then got munched. :D
The Combine don't have a stronghold over the countryside and villages because they're infested with headcrabs. The outlying areas were bombarded by alien creatures, forcing the majority of the population into the urban areas. So, basically, nobody has control of the countryside.

First of all, I think part of the reason you've never gotten all this help before, except in games like Rainbow 6, is that it's never worked before. How many games do you get to meet up with a squad of fighters along the way and simply let them do all the work?
