I've gotten shoved off and escaped plenty of times when I pounced on a survivor. It almost only happens to me when the survivors are melee'ing wildly when they are covered in vomit.
Definitely. After playing expert, advanced is a joke. Expert wouldn't be nearly as bad if there weren't so many idiots out there that don't listen/know what to do.
I liked COD4 SP and MP. Multiplayer was where I clocked most of my hours, but singleplayer still had some kickass moments.
Yes, they need to do away with the infinite spawning of enemies until you cross arbitrary boundary X, but there is still fun to be had.
Love4Fire and I just played Dead Air on VS. It still doesn't have climbing points and whatnot for the infected, but it's kinda fun to mess around on (especially after putting the difficulty on expert :D).
The server is still kinda broken. It's in campaign mode, but it allows VS play. It screws things up for the infected side because rooftops are not rendered and infected can't see or use many of the climbing points. Also, it's pretty obvious it's campaign because it has the campaign version of...

The reason the game looked so much better years ago was because it wasn't dumbed down back then. The biggest thing was that creatures' stats and behavior actually depended on how they were designed rather than by the part with the largest + stat on it.
The auto-shotty can take out 3-5 zombies in one shell when they're rushing at you. If you get to a decent choke point you can easily take an entire horde rush on without a scratch and without wasting a ton of rounds.
Not to mention you can fire it on the run without having terrible accuracy.

I can't decide between the two. Campaign is obviously more polished and balanced. The spawning and AI director in VS can be horribly unbalanced sometimes. Sometimes one team can get almost no horde rushes, T2 weapons right away, a shitload of pipebombs and be able to just stroll through a map...
Don't forget, if you're a hunter or smoker - keep an eye out on areas that permanently separate team members from each other.
